Case Manager

THE SELECTED CANDIDATE WILL BE OFFERED A SALARY OF $62,868.00 – $72,298.00. The Administration for Children’s Services (ACS) protects and promotes the safety and well-being of children and families through child welfare and juvenile justice services and community supports. ACS manages community-based supports and foster care services and provides subsidized childcare vouchers. ACS child protection staff respond to allegations of child maltreatment. In juvenile justice, ACS oversees detention, placement and programs for youth in the community. The Division of Youth and Family Justice (DYFJ) provides a wide range of services and programs for youth at every stage of the juvenile justice process. Our goals are to (a) build stronger and safer communities and advance public safety by preventing juveniles from entering the New York City's juvenile justice system, (b) provide therapeutic treatment and services to youth in our custodial care, and (c) help juveniles access the tools they need to leave the justice system for good. We strive to help families and young people by providing supportive services that are responsive to the needs of individual youth and families. NEST (Nurture, Engage, Support, Treat) Unit ensures that youth served by DYFJ (from detention to placement) are receiving comprehensive and consistent medical, mental health, case management, social and educational services throughout their involvement with our system. In addition, NEST organizes our family engagement and parent support work, and provides staff training on mental health, trauma, LGBTQ issues and Commercially Sexually Exploited Children. The duties and responsibilities of the Case Manager will include, but not limited to: • Meet with residents at assigned hall(s) to discuss their needs, concerns, progress etc. • Complete Case Management Intake and PREA for new residents at assigned hall. • Complete documentation/progress notes summarizing discussions. • Complete court ordered adjustment reports. •Communicate with parents on a weekly basis and complete parental notifications as needed. •Send emails and correspond with attorneys regarding their client as per the residents’ request or need. • Make resident referrals to mental health as needed. • Act as liaison to Mental Health, DOE, Group Services, External Providers and Medical staff. • Advocate for youth to ensure all residents get the services they are entitled to, while in detention. • Experience collaborating with multidisciplinary teams and building effective working relationships with internal and external stakeholders, including clinicians, educators, security staff, and community service providers, to support positive youth outcomes • Experience developing individualized service plans, monitoring progress, and coordinating referrals to internal and external support services • Knowledge of trauma-informed practices, adolescent development, crisis intervention, positive youth development principles, and evidence-based approaches for working with justice-involved youth • Experience coordinating with courts, legal representatives, and community-based service providers, and knowledge of the juvenile justice system • Strong organizational skills, with the ability to manage multiple priorities, meet established deadlines, prepare clear and accurate documentation and reports, maintain confidential records, and effectively utilize computerized case management systems • Bilingual proficiency preferred, with the ability to communicate effectively with youth and families from diverse cultural backgrounds

New York City residency is generally required within 90 days of appointment. However, City Employees in certain titles who have worked for the City for 2 continuous years may also be eligible to reside in Nassau, Suffolk, Putnam, Westchester, Rockland, or Orange County.
